A handrail has one job: to be grabbed by someone who is already falling. That is why the code cares less about how it looks than about whether a hand can close around it and keep moving along it without interruption.

Graspable profile requirements

Graspability means a profile a hand can actually wrap around and grip — a defined size range for round sections, and specific limits on the shape and dimensions of non-circular ones. It also needs clearance behind it so fingers can pass without scraping the wall.

The common failure is a wide decorative moulding used as a handrail. It looks appropriate and cannot be gripped, so in the situation the rail exists for, a hand slides along the top instead of holding on. Where a wide profile is wanted for appearance, the answer is a separate graspable rail mounted below or alongside it.

Continuity across landings

A handrail should run continuously along the flight and, where required, continue across intermediate landings so the hand never has to let go and find it again. Letting go mid-flight is exactly when a stumble becomes a fall.

Exterior stairs also freeze. A handrail that is merely adequate indoors becomes the only thing between you and a fall on an icy step, which argues for over-specifying rather than meeting the minimum. Metal rails get very cold in winter — worth considering for anyone with limited grip strength.
